I'm pretty sure (or at least, would hope) that when players are using "real" or "real world" in the context of a fantasy game that they're referring to the realism of the fantasy itself (or elements of the fantasy world). Even though games are fantasies there still exist levels of "believability" in them; just like in movies with fictional stories - otherwise they're just straight up bizarre like the shows (and games) you find on Adult Swim o.O
