I would like to say ghosting, but you did unplug the keyboard and I'm pretty sure you can't ghost in that situation. Try opening up a new Notepad or text document and see if anything is being written down automatically when the error occurs in game. Also key inputs can get ghosted when tabbing in and out of a program. Some of my games lock the alt and shift keys whenever I tab into them.

Do you have anything wireless hooked up to your PC? Keyboard, mouse, controller? These wireless receivers can interpret outside noise as imput by mistake sometimes. Has happened to me many times.

This is not called ghosting. That term was coined by LCD manufacturers to describe the delay of pixel changes on the panel which causes a blurring affect or "ghosting" as they coined it.

I don't know if this remains true for modern consoles, but back in the day a controller's joystick would "zero" on its current position whenever the system turns on or the controller is plugged in. It's entirely possible that the controller is resting in a way that has it pointing forward when you turn on the console, which means it would interpret "neutral" as "backwards".
