I have been reporting this exact same issue in game since day one. I was hoping the recent hot fixes would correct these issues/errors but no such luck. I only recently received what looks like an automated reply to post on the forums about the issue so that it could be taken care of in a "timely manner."

As annoying as it was to do so, I found a temporary solution to be to change the Squad Member's class. It breaks the glamour and stops the bug. I do not believe this should be an issue on launch especially since a handful of people were either looking forward to this or excited about it.
