3. Figured out it is a lvl 50 quest reward from "you're gonna carry that", but I already had one. I still don't know how to get rid of the item though.

Not an in game bug. You get that minion from a fate, and even if you have it already, moment you enter and participate in that fate, at the end you get the same minion. While it can't be sold, you can open your inventory and just drag and drop it, that will destroy the minion and you get it out of your inventory. problem solved.

Next time fill out the whole form and not just make a header and leave everything blank.
