I've cleared every normal/savage raid and ex trial released since 3.0, I seriously have no idea where you'd have trouble using holmgang when you can hit the target. I'm not aware of any boss with more than 3y hitbox radius (6y diametre), since that'd be the only possible case where you can hit the boss at max melee range but not holmgang it. Not to mention that when you're tanking the boss it'll follow you if you try to move away to max melee range from it, unless it's a stationary boss like a9s refurbisher, though I don't think he's big enough to give you difficulties to holmgang. Maybe you can tell me, because clearly the person I quoted didn't mention any specific example?
Edit: Well maybe some fate bosses or hunts are big enough, not sure though.
I probably worded it badly, should've asked why would you want to holmgang the boss if you're hitting apanda. Thanks for pointing it out.

the biggest boss i remember is Nidhogg. his hitbox is huuuuge
sephirot (last phase) and shinryu have gigantig hitboxes too. if we get an extreme version of shinryu this could cause problems with holmgang.

Oh good example, nidhogg hitbox in the first phase is definitely very big, but there's no reason to holmgang in that phase, and you should probably be standing inside the hitbox to get raid buffs. Even in the last phase I don't remember having trouble using holmgang but I most probably weren't standing at max melee range from the edge of the hitbox. I don't think you'll ever have trouble using holmgang unless you're consciously trying to stand as far away as possible from the boss though.
I may not be the most experienced war player, but from over 2 years playing, I have never even seen holmgang's range as a limitation or annoyance (granted, I never knew some ways of using it like pulling mobs to cancel their cast as mentioned earlier in this thread), since everytime I use holmgang I still want to hit the boss, so I won't be too far away from it. My reaction to this request is more like "why would you even want that?" instead of "no holmgang is fine the way it is" so I want to know if I'm missing anything.
Holmgang, like every ability, is based on distance to the enemy's hitbox. Basically, as long as you're in melee range, you can use Holmgang so you shouldn't have any issue with it (assuming you do want to keep hitting the boss).
I've been warrior since forever and I've never seen this as an issue.

I never seen an issue with range, however, maybe I suck, but I have died more often than not because of Holmgang. I have only felt the need for it in big pulls after going through all other cooldowns available to me, and the self-immob combined with short duration has more than once made me die to an AoE because the thing wore off leaving me with not enough time to move away from tons of telegraphed AoE.
I'd gladly take an increased recharge if some penalties got alleviated.
