If I remember correctly from what Yoshi said in one of the letters, basically it comes down to resource management. They prioritize new content over adjusting old content, even though he wishes we could fly in the older content it's not likely to happen anytime soon.
Gridania, Ul'dah and Limsa Lominsa? Yes, because they were originaly designed to be 1 map. Just add back the stairs and tunnels.
Ishgard? No, because that was not designed to be 1 map, but 2 seperate maps. It will need a lot of stairs to connect them.
Just look at the city maps in the new zone maps
What is that for a reason? You can't fly in Kukane and it is still a single map.
And even if they merge the 2 maps of the olders cities (except Ishgard) to a single map, there will be still no Fly mode within the cities.

I think because it was tied to PS3 limitation, from what I remember.
So, it's possible the old cities would be regroup in one map each.
Possible? Yes.
Will they likely spend the resources to do it anytime soon? No, not likely.
An unfortunate reality, as I also wish they would merge them. Kugane is so nice to navigate.
Possible? Absolutely. Worth the time and resources? Not at all. There just isn't a point to redoing the ARR cities unless 5.0 makes them relevant to the MSQ again.
They were originally single maps with thinly veiled loading zones in the form of long, enclosed corridors between the current sections.
That ship has sailed - 1.0's cities were all one zone (that was actually not even separate, the citystates were actually part of their surrounding region zones, so you didn't go to the 'Gridania' zone for instance, you went to Gridania in the Black Shroud zone).
Needless to say, this was one of the most complained about problems with 1.0 that Yoshi sought to change with ARR - Limsa and Gridania had long tunnels/bridges you had to spend like five minutes tromping down just to enter and exit each city, and Ul'dah... you couldn't even go through the front gate directly, you had to double back around side alleyways to get inside the city (you can still see where these alleyways are, look for the doors on the left and right sides inside the Gates of Nald and Thal in Ul'dah at the zone lines., they're naturally closed and locked but still visible - those were the entrances to the alleyways you had to use just to access the city as the actual main gates were permanently closed).
And because Ul'dah became the 'hub' in 1.0, all those players actually broke the zone in that the inn room became completely inaccessible which was never actually fixed - all SE could do was add a work around with a back door to the inn in another part of the city instead of inside the Adventurer's Guild which remained til the game was shutdown.
Either way, I do not see SE restoring the three original citystates to single zones as the change to split zones was actually a fundamental aspect of ARR's development from the very beginning.
