Character/NPC models won't show up

I'm also having this exact same issue. It's gotten to the point where sometimes when I login, some characters are not displaying. I have to relog in in order to get characters to display and they only display in the immediate vicinity around me and not all characters are shown to me even then.

For example, when I'm crafting I can see other PCs crafting effects but I can't see or target the PCs themselves. When I go to a shop or counter, I can't see the NPC even though the green dot exists on the radar, indicating that the NPC should be there. At times I can't see or target any players even though the radar tells me that I am surrounded by them. And this is in normally crowded areas. When I'm riding my chocobo, it will disappear and won't reappear even when I dismiss it and call it back, even though I can still ride it. Monsters that appear on my radar will not display in the open world and I cannot target them. This is at locations near the city where I know there are a lot of mobs that should be there. Aetherytes don't show up and can't be targeted. You can still see the sparkle effect that indicates where they're supposed to be however.

So right now, I can't see or target NPCs, PCs, or monsters unless I relog, and then it only shows me the models in the immediate vicinity. As soon as I venture out further, I can't see or target anything else.

My computer is an i7-920, Radeon 4870 graphics card, Windows 7 Ultimate.

I run all settings on standard and have turned off every extra feature I can think of.
