Not that it likely matters, but when I unsubscribed from FFXIV from the mogstation, I was kind of dissatisfied with the click generic response as to why you're leaving question.

As someone who was actually willing to provide in detail reasons for my decision to unsubscribe after playing continually since 2.0's launch, and what measures might make me consider returning, I was kinda disappointed not to have that opportunity.

I'd suggest having an optional section for players to leave detailed feedback if they feel so inclined. Or at least an optional lengthened survey for players who wish to do so.
