It really depends. If the party has had a lot of deaths before the LB3 even builds up, they may silently decide to hold the LB3 until a certain percentage in the event a healer LB is needed. This is also the case if a group wipes several times due to people using the LB3 too early, which, in Susano Ex's case at least, can happen if the party is not that great. Typically, I only mention saving the LB for healer LB if there have been 2-3 wipes prior where melee used the LB3 at, say, 30%, and then we wiped soon after each time. If the party is good to where there are no deaths and there's no need for a healer LB, then the melee should be using LB3, followed by casters, followed by ranged (BRD/MCH).
Some fights, people wait for a certain time to use the LB, since it does lock the caster in place. For Susano, I know that, as a healer, I wish the melee would wait and LB after the Rasen Kaikyo's appear, because a lot of the time, they LB right as the second set of rock gaols appear, and Rasen's appear shortly thereafter, and they die either mid-cast, or right as it finishes. There are also times where a melee decides to LB, and they get marked with one of the purple Levin markers. And since they're locked in place, they end up electrocuting the entire party, which requires unnecessary AOE healing. So it could be that melee are waiting until mechanic-heavy phases pass to use the LB3.
There is also the possibility some players are waiting for another melee to LB. Or that they simply are tunnel-visioning too hard to even realize there is an LB3.
And then, there's the melee that want to wait until that last 1%, because some people think that using the LB3 at 1% to kill off the boss is some sort of DPS gain when, usually, waiting that late is just a waste of LB damage.
