Is Materia VI working as intended?

I've read pre-stormblood that you can only slot 1 Materia VI on each equipment, and that's on the first slot, and succeeding Materia VI melds will fail, yet I see people donning multiple Materia VI in one piece of equipment.

Is this working as intended?

Don't know how much before Stormblood you read that, but from April Live letter:

Q: In Patch 4.0 we expect equipment item levels to skyrocket all at once, but will there be higher grades of materia introduced?
A: Yes. The name is tentatively known as High Materia V and we plan on making them easily obtainable. As they are quite powerful, we plan on restricting advanced materia melding to one time only.

The restriction applies only to overmelds.

From the patch notes:

Grade VI materia has been added.

* Attempts to affix grade VI materia will always result in failure when used as the second or later materia in advanced materia melding.

So, based on the wording above, you can only have one overmelded Grade VI, which means visible pieces can have up to 3 Grade VI (2 guaranteed, 1 overmeld) and accessories/belts can have 2 Grade VI (1 guaranteed, 1 overmeld) materia.
