At this point in time, the expansion could of been titled Stormblood Of The Mouth. /s In all seriousness, I've seen this at least five times in the story so far, and it's time for a new gore effect. Make a soldier fall and get impailed on a spike, make blood for the abdomen, just do something new SE. It's making your injuries/deaths less impactful.

The game already deals with some grounded and serious topics, I don't know why gore itself is avoided at such a great length - we all know what being stabbed entails.

I beleave it has something to do with laws of countries it is in, if they added in a lot of gore there is a high chance it wouldn't be sold in some of the countries they currently sell in and rather then re animate cutscinesnto remove impairment they can save time and money by doing one cutscenes for all

In the US there is a thing called the ESRB (Entertainment Software Rating Board), where they rate games on if they would be appropriate for a certain age of consumer. If they were to implement something that you suggested the game would change from a something like a "teen" rating to a "Mature +17", so every game is walking that thin line before a lawsuit comes their way so they are careful just how much blood and gore they would put into the game

some countries out right ban a game for content. Australia is pretty infamous about it. The US was gonna ban outlast 2 till that scene was changed.

It's a cumulative thing, the ESRB rating (and on the subjective side as well).

Some not explicit references of things like that, but without a lot of blood and gore? Teen rating. Take away the sexual references and have more blood and gore? Also a possible Teen rating.

Both in the game? That could either be right on the edge and still be Teen rated, or could easily push it into a Mature rating unless things are toned down a bit.

Language also has an impact on the overall rating as well, and this game isn't exactly PG in that regard either. Anything they add that fits in one of those kinds of categories they do have to be careful with, to avoid an increase in the rating.

That being said, we don't actually know where they fall in the rating scheme for any one country's rating system (ESRB is going to be different than PEGI, for example), and it could be more development-based things, like custom "wounded" models of gear (blood appearing on other places on gear consistent with a wound is the more likely addition versus physically visible wounds) or characters not necessarily having a place in development pipeline.
