1. Quest: A Dash of Green: I have collected all herbs from town and got the "step complete" message for them, and the turn-in NPC Ahelissa has the green Quest Complete flag active, but I cannot turn the quest in because the herbs are not in my inventory. Again, the quest acknowledged that I gathered them, I just did not receive the key items so I cannot progress.
2. Quest: Digging Deeper: Exactly as above, but I didn't get the ore from the mining points, even though the quest says I completed that step.
3. Quest: A Missive From the Far East: Also as above, except I cannot turn in the maiden's katana to Brithael to complete the quest even though he has the green Quest Complete bubble over his head; I completed the steps but did not receive the key item.
There seems to be something wrong in my game that is preventing me from getting any key items at all. This has stagnated my progress in the MSQ and for the Mining and Blacksmithing quest chains.
