Please let me share an update from the User Interface Lead Minagawa in regards to the cause and solution for others that may be affected by the issue.

This isn't the first time new functionality had lacked a keybind for those of us with old saves, though it's one of the more high profile ones. It would also be nice if there was a way to revert all unbound functions to default, without resetting custom keybinds to default.

Hmm... It could actually be that I haven't learned it. Have to check. Very embarrassing if that is the case

Not sure if necro..but what the hay:

1) Assuming you have unlocked diving in Stormblood (part of the main story)
2) rebind it somewhere else, without ctrl, to see if it works. Like the “v” key.
3) Does your keyboard have sticky keys/ghosting issues (too many keys at once = irnogred keypresses). Check that.
4) make sure you are trying to dive in open water and not shallow pond/coast
