Suspicious activity has likely occurred on your account (different ISP, strange computers, strange browsers etc). You need to contact Square Enix support (recommended to use the phone option through the Mog Station) to get this sorted. You maybe required to to send scanned copy of documents to prove your identity which can take a while to process.

To prevent this from happening again in future, use Square Enix's one time password system.

Except, I use their one time password system. This is yet another example of SE having massive problems with their system and leaving a group of players out in the cold. I've seen bad launches before, but nothing of this scale from FFXIV. Maybe I just expect to much out of them, but the fact that not one dev, not one GM has responded to this thread is disconcerting.
