I seriously doubt the empire is going to take the liberation of two of their conquered nations sitting down, so I imagine the post 4.0 story is going to involve their retribution and build up to the eventual decision to go to war against them proper.
so here is my theory:
it seems that the theme for A Relm Reborn was "earth" and the theme for Heavensward was "sky". this is mostly presented as the means of which we travel, and it seems that Stormblood is mostly leaning toward a "water" setting.
thus, i think the next expansion will take place in a "fire" like setting, but not just in the form of volcanoes. from the top of my head, im thinking desserts, savannas and strangely an industrial theme with machinery-based and engine-based technology (perhaps something with wheels).
in conclusion, it is my belief that the next expansion will take place in either the Garlean empire, or most likely, The Kingdom Lucis, industrial era.
a continent which does indeed have a volcano and savanna landscapes.
Next one will be Hellwind, which means we won't ever be able to log in and play the expansion because
a NPC that is a literal block will pop up on the expac and stop you, unless you click 90000000000000000000000000000000 times in 1 second to get in.
If you don't do it, you're 90kd, can't move, black screen of death, Pipinbahn'd, que back in 69 billion to try again.

My theory: Garlemald is obsessed with Allagan technology (Ultima Weapon, Omega, Azys Lla). After losing their conquered territories, they'll want to unleash a great power over Eorzea and Othard to reclaim them again. That is why they'll look for dangerous Allagan creations. But where? Easy, one of the most unknown continents of Hydaelyn, the one destroyed by the same ancient empire: the forgotten wild lands of Meracydia.
Garlemald will invade the forgotten land convinced that many Allagan technology are buried there. So we should go there. And, if there is still people living there, to them we and Garleans are the same: bloody enemies from Allag.

I'm not being very nice about this. I think 5.0 will be about getting the rest of the nations to attack and take apart Garlemeld. The aftereffects being treating the garleans like scum and "giving them what they deserve". thus heightening their desperation, but they have no god to appeal to unlike everyone else...until one gets given to them via Ascians.
I hope 5.0 won't be this. I actually don't want the whole world reduced to a bunch of republics all content in their poverty or continuing problems. I want the empire to survive, even if it needs to change. I don't want destruction to be okay simply because Eorzea does it.

Prediction One: Old Sharlayan will lose control of their voidsent and, request the adventurer. Everything will fall apart. Prediction Two: Garlemald will go to Meracydia for some reason, and Thavnair will be our first stop. I do believe that the Thavnair embassy is signs of our future. Why is such a prominent building in Kugane there? Prediction Three: We will bring the war to Garlemald, and unlock more eastern zones, such as Naxia.
it'll be garlemald most likely, it's the only location we know of that we haven't been to yet.
My guess is that it is going to be a Garlemald/Sharlayan Expansion for 5.0 as the beginning of 5.0 involves finally building that united force to invade garlemald by convencing Sharlayan to join the Alliance then the WoL gets captured as a Prison of War. Wakes up on a Prison Transport called Doomtrain that arrives in Garlemald and from here the WoL must find a way to escape Garlemald while learning that not all Garleans are "muhahah" evil and how ignorant the civilian population is about the crimes commited by the Garlean Empire outside of Garlemald resulting in the WoL finding Garlean Allies by revealing the truth and a Garlean faction that are aware of the crimes and do not support Garlean Empire's cruel actions. Then the last half is the war in Garlemald with the final dungeon being the Capital City of Garlemald as the Warrior of Light and the rest of the invading Alliance forces arrivies in Garlemald on Doomtrain.
Then 5.X involves dealing with the aftermath where issue erupt as garlean citizens are being mistreated by people from Othard and Eorzea because they are Garlean resulting in the Othard and Eorzean people acting no different from how the Garleans treated them despite many citizens were ignorate of what was going on outside of Garlemald because the only news they get was that the Garlean Empire was "civilizing" the "savage" countires thus excluding all the other information about their cruel actions from the majority of the Garlean civilian population
Also they release 2 New Jobs being the Gunblade Tank Job and a Dancer Healer Job. No DPS Job will be released because they want to only release 2 new Job per expansion and 4.0 already introduced new DPS jobs already.
