I've been getting this error lately.
Whilst it doesn't happen in regular dungeons, it does occur rather frequently in EX Trials, Raids and worse, Hunts.
Basically, if there are more than 4 players doing constant attacks I get this error.
My GPU is a GeForce GTX 880M. It isn't overclocked and the temperatures for it are in the normal range. My OS is Windows 7.

I tried some of the various fixes suggested in this thread, but only one worked which was lowering the clock speed on my graphics card. I have a Zotac 1080 Ti Amp Extreme which comes with a rather aggressive overclock. I dropped the clock speed by 25 Mhz and haven't had a crash since. Previously, I had tried clean driver installs, overlay removal, Games for Windows disabled, ect. I would make a change and play until I got the directx error and move on to the next possible fix. Lowering the clock speed on the graphics card is the fix that worked for me. This is the only game I have that requires me to do this, but it works and I don't mind a 25 Mhz drop as long as I'm not getting the directx crash. As a side note, I never crashed when playing in directx 9, but the direct11 version uses more resources and pushes my graphics much harder than the directx 9 version.

I found a resolution for the Nvidia bug issue. I bought a AMD card and took the Nvidia card out. A 400 dollar fix for a bug. However it is technically a better card than my 780. I have not had one single crash last night raiding and game ran silky smooth on DX11 on maximum settings in 1920x1080 resolution.
I have been talking back and forth with Nvidia's team and they currently have no resolution coming for this any time soon.
If you have the luxury to do so and you are a heavy FFXIV player frequently experiencing this, I would suggest going over to AMD series cards. They did have the bug once I believe on 380 cards in particular but they found the bug and updated it. If Nvidia can't find this bug after nearly four months of going back and forth with them, then I lost hope for them at this point. I feel like I went to the dark side with AMD, but ahh well, my games run incredibly better and no crashes on FFXIV. I know this has been suggested a few times, but I also found that reducing the overclock on my Sapphire Nitro OC R9 Fury has stopped (or at least drastically reduced the frequency of) this CTD. I dropped the max core clock in 10Mhz steps until the crashes seemed to go away - now with 50Mhz knocked off, I've been running the game for 12 hours and counting without a hitch. It's worth trying, even if you have to lower your settings slightly to maintain a smooth framerate.
The game now occasionally freezes briefly instead, but I assume whatever causes that is what previously led to the crashes.
P.S. for AMD users, WattMan in Radeon Settings will let you control clock speeds per process:
