Why am I always losing aggro on Ultima Weapon?

Cool. Much appreciated. I thought I was doing pretty well but obviously I still have alot to learn!

Hardest part is getting everything I want to use on my bar. Im starting to feel like my PS4 controller is holding me back :/

I use 4 bars:

1 is my main, which has my Butcher's Block combo.
2 has my Storm's Eye Combo
4 has my Storm's Path combo
3 has my stances (Defiance, Deliverance, Unchained, Infuriate)
8 has my Defensive CDs (Foresight, Vengeance, Inner Beast, etc)

The combos for 1, 2, and 4 are all on R2+X, Square, Triangle while the other slots on that bar houses abilities like Berserk, Limit Break, Provoke, Brutal Swing, etc.

My defensive CDs, as noted above, are on Bar 8, but some are macro'd. For example, Holding L2 then holding R2 brings up Bar 8. I will press X twice for Foresight+Raw Intuition, or pressing Square twice will give me Thrill of Battle+Convalescence. Why the two macros? Because the cooldown on both abilities (Foresight and Raw Intuition, for example) are the same.
