Consider in Japan that cross-dressing is actually on the common side; there are lots of boys who cross-dress as girls and pass for it as well due to their natural appearance, and its generally accepted.

This. Sort of. I would say most times I go out in Tokyo I see at least one older male dressed in school girl or lolita attire and no-one bats an eye. I don't think it is quite as common as you would have people believe, but definitely more common than in the West.

Edit to add that the cross-dressing I have encountered isn't just by boys with feminine features so it is obvious that it is a man wearing women's clothes.
