Well plenty of hats have ear pockets on miqote, or that weird evenstar tail sheath. There's gear that looks different depending on if a male or female equips it too, like how WHM AF pants become a panty-skirt. Granted the hempen gear is probably the most different looks on a single piece, but I don't think that means it's made differently.
The hats are still the same model, with a slight modification for the ears, same goes for the evenstar chest and Miqo'te/Au Ra. The hempen set uses entirely different models based on which race, so it's 6 different models on a single item. I think if it were something simple, they'd also be able to use this for multiple glamours on the same piece of gear.
All gear has separate models for each race, since the races have different body shapes (most noticeably in the case of lalafel, male Roegadyn, and male Au Ra, but there are slight differences in all of them). In most cases, those models all share the same design, only changing shape to match the character model, but they can have more differences than that, like the ear pockets on certain hats and hoods for Miqo'te and Elezen. Significant design differences are more common between genders than between races, but work the same way.
(What I don't know is why there's a separate Hempen Camise and Hempen Undershirt. They're already changing style by which race equips it, why have different items per gender rather than changing by which gender equips it?)

Actually you're wrong about a few things
1. The only type of gear that's genderless is ilevel gear from npcs or dungeons
2. Almost all glamour gear is separated by gender
3. The reason butt slider wouldn't work on any of the glamour options is because of the first point. The only gear that adapts to sliders are the race and gender ones like hempen (they know both requirements to be true for sliders to adapt). That would mean they would have to do that for all leg gear/glamour in the game.

I am a little bothered by the fact that Koji completely botched this question during the Q&A... no one was asking for an underwear slot, so I am not sure why they even thought that was what the question meant. Does anyone know if there's a way to bring attention to that mistake?
I'm not entirely sure Koji messed up the question. I think he said something about using the pantalettes with all races. I assume Yoshi misunderstood what pantalettes meant.
True enough. I only hope that someone tried to clarify it, since that question got completely misconstrued in its entirety.
