Worldwide. Most games bloat their numbers like this hence why they release shipped copies instead of sales figures. WoW used to boast about their 10+ million accounts, but stopped after the mass exodus of people.

The last few minutes are great but the trailer as a whole doesn't grab me the way many previous ones have. I think it's the music to be honest. Usually these trailers open with a dramatic hook and a new piece of music but this one opens really slowly...
