It's from the level 50 DRK quest "Our Answer". ( NPC named Esteem )

It's the lvl 60 set that used to be bought with esoteric tomestones. Now it's sold by the centurio seal (Heavensward hunts) vendor, at 520 centurio seals for the full set, and 140 seals for the sword.

Chaos Set and Deathbringer, sold by the Centurio Seal NPC in the Forgotten Knight where DRK Quests for levels 50-60 are received.
