Depending on where they live, I'd imagine there are outhouses or similar facilities. It's most likely they do it outside and in bodies of waters like what people did in the olden days.

Actually it's called that to deter people from fishing there because it's a super profitable fishing spot apparently.

On topic though, I just figured we pissed and shat in rivers cause of the lack of toilets everywhere.

Ugh, it's just like the stories my friend told me about his time deployed in the Philippines. If it looks like the place you're in wouldn't have a dedicated and well-kept water reservoir for showering, it's likely pulling the water from the river that everyone uses for every daily need (including drinking and pooping).
