Then you have not looked hard enough. I seen at least one or two people at patch launch complaining about how RNG the dungeon was or how the door cutscene took too long.
Like I said, could be based around a mechanic. Maybe Alexander's ultimate attack. There is a safety bubble, but only one person can fit inside. Healer goes inside, everybody else dies, healer LB's to bring everyone back.
I really hope not doesn't seem as epic when you deliberately have to die, sack mechanics have been in a4 and a8 now mainly because it's easier but it should never actually be required to die that's just cheap.
Agreed. Limit Breaks are a clutch if you done goofed (healer and/or tank) or squeeze out that last bit of damage (DPS). Requiring players to die just so the healer can use a Limit Break to save everyone after a mechanic is not good game design and it also means the fight becomes unwinnable if the healers eat the floor before that attack goes off.
Heck, I watched Retsupuare riff a Super Mario 64 hack where in order to unlock a panel in the wall to get inside the castle, you had to have Mario die so he could respawn inside a stack of boxes and talk to an NPC to unlock the panel. It makes no sense, just like that theorized Alexander mechanic.
Removal of furniture destruction on most primal furniture. Summoning bells in Gold Saucer. The only things that'd be considered QoL in the notes would be that moogle quest having its npcs moved, the door cutscene in aquapolis being changed, tooltip showing full name of long orchestrion songs, the materia melder for estates, that compact version of the lv 6 aetherial wheel, and using food/meds while mounted. Technically not even that last one. And none of these are that significant. But then that's what QoL generally means. The only big example of QoL I can think of is when they added item sorting back in 2.1, was pretty crazy it wasn't there in 2.0.
And a plethora of display changes, most important of all:

Can someone please for the love of sweet lord Baby Jesus tell me how to rank up to First Lieutenant? He keeps saying I'm maxed out, but this patch says you can now rank up. I don't see a quest anywhere, so I am completely lost.
