I question whether I'm playing a PvE game with PvP mixed in, or I am I playing a PfG.
Whoever impresses me, really. I usually do dungeons as a healer, so if the dps are doing great, it usually goes to one of them. I rarely give tanks commendations unless the dps are especially bad or tank is super impressive in some way. Sometimes I'll give commendations to those that are friendly or helpful. I once gave someone a commendation for complimenting my sunglassses and another for thinking my search info was hilarious, so complimenting me is a good way to do it.If I can't decide, I'll choose the lala if there is one because I'm biased or whoever has the best name/glamour.
Maybe my process is too simple. If I am a DPS > DPS or Tank<->Heal. However all you have to say is "Hello" or "Hi" just something to start the dungeon or w.e off rather than being completely silent. If only 1 person says hello, they instantly get the Commendation as long as they are not completely helpless or rude for no reason and no "o/" does not count in my books.
I give them to people who don't make me feel like a npc turret in dungeons. That is to say, that most of the time I don't give them out.
On a jp server it's actually hard to choose; mostly everyone plays well. I'll normally give it to either the tank or heals by default. But if by the off chance the tank is bad (i.e. a paladin that doesn't use shield oath and I steal agro on blm with thunder) or a healer that doesn't heal, then yes I'll give it to a dps.
Interestingly I get a lot of commendations when playing summoner. I don't feel like I really do anything special on it either.
Yeah, I will not give my commendations to a DD who doesn't use available LB as early as possible (to start charging it again) to the biggest possible benefit. I'm one of the annoying people occacionally calling for others to LB because most of the time I see LB go completely unused for the whole dungeon.Telling people to LB is orderly acceptable as the standard operating procedures for this aren't as prolific as you'd think. Furthermore, everyone contributed to filling the gauge, they should have some say in how it's used. Lastly, there is absolutely no reason at all to hang onto it until 15%. None. You risk dropping damage on the ground if you wait too long and that is a waste.
On topic, I will try to give my commendation to the player who I felt worked the hardest for the party and did their job really well (unless they have a really lame name or glamour).
I also check for who is pulling their weight well for the party, if everyone has done a good job, then I give it to the friendliest player. (Who said hello/goodbye, small talk, made me laugh etc.)
If more than one player hits all of the above, then I try to give it to a dps as tanks and healers seem to get them far more often.

-If im healing, im giving it to Tanks doing their jobs well enough to let me DPS most of the time.
-If im Tanking, im giving it to healers doing their jobs well enough to DPS most of the time, without having to fear for my life.
-If there is a Roegadyn, insta commendation regardless of the job. Although if there would be a Roebro and a Femroe, i'd commend the Femroe.
-If one player is new, asks about strats and applies the advices im giving him, i'll commend him.
-If there is more than one new player asking for strats and applying my advices, i'll give it to the player who performed best.

I give someone a commendation based on what they are doing during a dungeon run.
1. If someone is kind towards their party member and offers tips and tricks on how to improve their gameplay, they will get a commendation.
2. If someone is funny and makes the dungeon run more alive, they will get a commendation.
3. If someone is excelling at their roles during a run.
I'm probably the most heartless when it comes to comms.
I used to just divvy them out because challenge log free exp, but daily roulettes and anima grind pre-210 nerf has left me much more jaded when it comes to comm-ing players. I'm also not a social person. I don't use the DF to make friends or enemies. I'd be alot more open to it if it were relegated to server, not data centers. Honestly. It's a problem I don't say hello/goodbye? seems rather petty. oh well. you think i'm rude for it? cool. chances are we wont see each other again... ever. I'm not friendly. I'm off of my 9-5 and tired from doing my co-workers workload for them to get paid my wage. sue me, I don't have much.
That said, I tend to pay more attention to enmity bars as a DPS and cast bars as a tank. Something needs to jump out at me to think "huh... this guy deserves this thing that might do something some day".
things die super fast and it shaves several minutes off a run? I'll comm the DPS that does more.
no one is in immediate danger of dying and i hear the beautiful shimmer of holy? WHM gets it.
I'm doing a full bursty rotation with lucky crits and the enmity bar doesn't even budge past 50% and the tank isnt in tank stance on the boss? DF needs more tanks like that.
needless to say, I don't comm much and I'm terrible at parties. don't invite me. and GET OFF MY LAWN!!
