A few questions then since this doesn't seem like a SE specific issue or more people would be having it. (seems more like a system issue)

Has your system always had this issue since you built it or is it something that just started happening recently?

What are the rest of your system components? Motherboard, Ram (Amount, Speed, Brand) GPU Model/Brand.

How are you connecting your GPU to your screen? DVI, miniDP, DP etc?

If this just started happening recently has there been a temperature increase where you live?

When you do your clean windows install are you installing all your system drivers from Downloads or from the Motherboard CD?
