    So I never complete Levi EX

    And yet the questline for that fight is complete?
    I'm fairly sure I'd remember running a trial as extensive as LEX

    Is this something that just took place with the recent update or what because I'm playing catchup on my EX primals and this just struck me as odd

    My first LEX clear ended up being joining an in prog that pretty much had the fight one even if I didn't join...which is good because when I zoned in it was right before the body slam which sent me into the ocean. Possible it happened to you as well if you really don't remember clearing it.

    The quest for levi ex is completed when you unlock the fight, rather than when you complete it. It's the same with some of the other ex primals too.
