Anyway, back to the original topic, I suspect that what the OP saw came from this:

Though it wasn't clear from that note, apparently if a crafter moves to another server, or someone with crafted items in their inventory moves to another server, the server with the items ends up not having a record of the crafter's name (since they're on a different server), so all those items end up with that default message where the name would go. I'm not sure if there are other reasons as well for the system being unable to display the proper name, and end up showing "Obtaining Signature" instead.

I've also heard that at least on some servers, players have decided to joke around with that message and create a character actually named "Obtaining Signature" (who may or may not end up crafting stuff, which makes that name on crafted items a bit ambiguous if they do).
