No one's mentioned using Fight or Flight just after Shield Lob. It pulls just enough hate on the whole pack as they come in so that you don't get singles peeling off and they're grouped up nicely for CoS/Flashes. I read somewhere that when DPS pop cooldowns as you pull it can peel mobs onto them unless you pop a offensive cooldown yourself after the Shield Lob. It works for me, anyway.
Also if the group has a summoner or other class that does damage to everything as a matter of course I'll also tab through the pack- like doing FB/SB tab RoH tab Flash FB/SB tab RoH tab Flash etc. Or I have two DPS focused on two different mobs I'll flip back and forth between those two while dropping CoS/Flashes in to keep healer hate off the rest. It's adapting to what your group is doing and responding accordingly.
