Nothing is ever gonna top that. And that's when they had guts to do crap like that. Now when they do that sort of thing in XIV and even in recent games it feels rather forced. Or they backpedal completely.
In my opinion, I was grateful Nanamo didn't die, but at the same time, I have to agree with Fenral on that the potential for radical chance is there but will never be fully realized an I think the fact that 3.0 is not needed for 4.0 is related, (Though I'm not sure if its the cause or the effect of that)
Also I blame the Death of Superman Comic Book arc for the "Death is Cheap" thing happening in media in general

Rather than being condensending, actually read why some people are bringing up the issue. For example, maybe it's been overdone to death that characters drop dead left and right? Maybe we're sick of characters dropping like flies and then seeing them go "LOL I FAKED MY DEATH"? Death is such a joke in stories now.

That would be my complaint, as well, and the Nanamo fiasco is probably the worst offense. Her death was shocking and unexpected, and left the potential for some amazing storytelling during Ul'dah's subsequent political upheaval. Except, no - it was just a cheap cliffhanger to get folks psyched for Heavensward. As much as I've loved the Heavensward story, I was nothing short of insulted by the offhand way everything in Ul'dah was tidily returned to the status quo, complete with Nanamo being given a gentle chiding for her silly naive notions of political reform.
I'm not in favor of the story descending to GoT's hilariously overblown levels of life-is-cheap, where anyone with power over someone else would cheerfully slit their throat for a nickle, but when you do go ahead and plan a shocking twist of this sort, follow through, dammit! This may not be A Song of Ice and Fire, but neither is it X-Men.
I do hope we see at least one major character die and stay dead. Until that happens we can safely assume that every leader figure and prominent Scion will just survive everything...and that unfortunately means that secondary characters will continue to be at risk of being built up and then killed off instead. It's possible that Minfilia isn't dead but it's left so vague that it's equally as likely that we'll eventually save her and that she'll be brought back into the plot.
I'd be less bothered by it if the injuries that major characters sustain were meaningful and impacting but...Raubahn, Thancred and Y'shtola seem to just rise above everything. Heck, Nanamo being left sickly and weak after the poison would have been interesting but she's perfectly fine as well.
To their credit Aymeric's injury was lasting - though I didn't like how he got thrown around by Raubahn. The developers do seem to listen to feedback too so hopefully the stakes will be higher in 4.0.
