Unable to become a Mentor

My original post got moved to insufficient information where I can no longer post to provide more information. So im posting again.

On patch day, I met the requirements to become a mentor, I became a mentor, and the next day I was no longer a mentor.

When I talk to the Smith NPC. He acknowledges me as a mentor, but then "Become Mentor" appears in the list of options. When I try to become a mentor I get the error. "unable to accept Mentor Certification at this time"

Its like the game has me flagged as a mentor, but at the same time, doesnt.
