And when I say properly, I mean dps doing damage, tanks keeping aggro, and healers making sure no one drops on the floor - and everyone actually performing the mechanics, which includes dodging AoEs.

Beyond that, it's up to the individual how well they decide to expand on their base role and duties. If that means for a healer to dps a bit between heals, go for it. But it isn't required and the attitude everyone has now with demanding healers play a dps role when that isn't their job description is ridiculous.
That really depends, if for example one dps is contributing less than would be required to do content, or is hampering the other three in the team, they should pick up the pace.

You are in a team, and you have to make some compromises as part of that team. If that means, the high end min/maxer being ok with someone doing acceptable, but not max damage, that is what it takes, but, at the same time, the Ice Mage should use a proper rotation to get through.

Generally in DF I go by the games meta, it is the most common goal. If I want to take it slow, and explore, I will create a team to do it with, rather than joining roulette and looking at all the pretty scenery while others are trying to get through.

Its fairly easy to get a read on what your teams goal is in the course of a run and everyone should make compromises towards that goal.

One point though, the high end min/maxer does not hamper the team by performing well in their role, the Ice Mage does.
