Over on reddit, someone started a thread with this title. I made my list, and having seen how long it got, I wanted to share it over here. I'm hoping to get some good discussion on these points as well as any other gripes other people have. I'm also hoping that some of this may make its way to the dev team.
1. Gathering - I don't like that I need to swap jobs and use skills for gathering. I'd much rather a system like in GW2 where it's just grab your tool and go - it's much more of a leisure activity.
2. Crafting - I don't like that you use skills swap jobs for crafting either. I'd much rather a system like Wildstar that allows for customization of stats in what I create, but is a simple interface.
3. Interface - I don't like that you get errors when you attempt to interact with two interfaces at once (currently occupied etc). This extends to teleport interrupts due to trades, and constantly having to select everything multiple times with confirmation popups etc.
4. The over-reliance on DPS checks with the ToS against parsers
5. Hunts design encourages zerg mentality and early pulling
6. Healer LB gives maximum weakness, making it effectively worthless - as once used everyone dies in 1 shot to anything
7. Game encourages you to play all jobs on one character, but then limits your ability to gear multiple jobs
8. Game limits you to gear one job, makes content favor one job, balances this by making jobs homogenized - rather than adding the ability to gear multiple jobs
9. Secondary stats are worthless and uninteresting. Having 100 CRIT vs 100 Spell Speed will net my SMN slightly less than 1% increase in DPS (or a movement from 1343 dummy parse to 1356). Secondary stats just blanket add to damage, adding no real choice in the matter
10. Yoshi's response to critiscim is usually 'git gud', 'ps3 limitations', 'server limitations' or 'we understand you think that, but you're wrong.' Instead of 'we appreciate your feedback and will continue to try to improve our services to better suit our playerbase'
11. MogStation - everything about this.
12. Maintenance requires a complete shutdown of the server for hours. I wouldn't have realized how annoying this was if I hadn't come from Guild Wars 2 - where you just get "a new build is available, please restart the client" which you could then do when you are available to.
13. 60 hard cap on dots present and 30 cap on dots visible on enemies in a very DoT heavy game
14. A heavy focus on giving the sense of forcing people to redo old content, rather than making content that people want to redo
15. A rapid jump in ilvl's which results in a lot of fun content being outdated practically on release (i.e. you could easily get your 180 law weapon prior to going in to Bismark Extreme)
16. They continuously add a plethora of tokens/currencies to our inventory rather than just adding it to the currency wallet
17. A complete lack of a wardrobe system that remembers glamours, resulting in an excess use of inventory space and server usage
18. A desire to make crafting more desirable than it is - crafting items for Relic was random and just a way to transfer gil to crafters
19. Materia acquisition through spiritbonding is not fun, and melding through crafters is frustrating
20. Mentioned elsewhere, but the inability to communicate on LS/FC when in PvP and to send tells / receive while in Dungeons/Raids/Trials
21. A limited housing system that doesn't allow the majority of players who want to take part in it, to take part in it
22. A limited housing system that doesn't let you customize the look of your houses exterior build
23. The entire retainer system, I'd rather have a bank and just be able to both buy and sell stuff from the Market Board
24. Server side calculations result in a lot of actions (animation and cooldown) going through without the proper effect - i.e. Benediction, Hallowed ground used and the tank dies anyway despite cooldown being triggered
25. Server side calculations that result in needing to wait for things to register before using something else (i.e. place Bio, wait a sec before Bane/Fester)
That's all I can think of for now. What bugs you?

Thanks for all the responses so far! I'd like to add that this was intended to be more about what bothers you in regards to the game itself, and not to do with your interactions with other players. If you think there is a game design that encourages the poor play by other players, then go ahead and include that for sure. For example:
26. That there is no warning before Castrum and Praetorium after the numerous complaint threads, regarding the cutscenes and finishing the dungeon. They could simply add "Castrum and Praetorium are both Cutscene heavy duties; however, due to the nature of Duty Finder, many players find they are forced to skip these cutscenes to keep up with the party. We recommend you create a party finder group to fully enjoy the story."
27. A system that makes things very easy for the average player (main story line quests and duties), and thus encourages people to not perform well in more difficult content (end-game) and often results in people who seek to be carried
