I don't have this, I feel there is too much potential for an error (like hitting cleric stance twice in panic and having the bars change 2x but cleric stance changing 1x due to CD).
I personally place all of my DPS stuff for my healers on CTRL 1-6, CTRL V-N, ~ and Shift~
PS4 controller and keyboard on a PS4 console. Most of my menu commands and out-of-combat abilities are on my keyboard, and my in-combat abilities are on my controller. I play in my living room on my couch with a coffee table, so I don't exactly have easy access to my keyboard. Aside from that, on my warrior (since it's the class I play the most) my keybinds go like:
L2 + ∆: Flash
L2 + O: Foresight
L2 + X: Bloodbath
L2 + ͏͏͏□: Convalescence
L2 + ↑: Company Chocobo
L2 + ↓: Gyshal Greens (I use teleports and chocobo porters a lot, so I have to have this keybound)
L2 + ←: Defiance
L2 + →: Sprint
R2 + ∆: Overpower
R2 + O: Heavy Swing
R2 + X: Skull Sunder
R2 + □: Butcher's Block
R2 + ↑: Brutal Swing
R2 + ↓: Maim
R2 + ←: Tomahawk
R2 + →: Fracture
It's what I find most comfortable while causing the fewest mistakes.
This was about 3 months ago, but nothing has changed much really.
My general finger position is (in order from pinky finger to thumb) SHIFT - A- W - F - SPACE. I main BRD only, so I made the setup so I can maximize my BL procs.
I also do use a Logitech G502 mouse, but I only have Flaming Arrows (5) set to a mouse shortcut. Just makes it easier to directly place the aoe.

I play with a Razer Naga mouse and Razer Tartarus gaming pad. All my key binds are set up from 1-12 + shit and alt. :3 Makes playing a lot easier for me. ;3

I have a keyboard and a Razer Naga: 12 buttons under the thumb, scroll button and wheel, and 2 buttons near the index finger.
12 buttons are for 3 action bars used on their own, with shift or with control. Alt is for the pet bar.
Scroll wheel for camera distance and scroll button for Mumble.
2 buttons near the index finger are for mount roulette, RP walk and auto run.
WASD for movement (legacy).
Q and E (+ shift and ctrl modifiers) for certain job actions and Sprint.
F for focus target (tank if I'm healing), tab for enemies.
T for target's target. I forgot what R is for... Nearest enemy?
Space bar for jump of course.

I tried doing more, but the other extra mouse buttons on my gaming mouse kept derping or just not mapping o_Oa

Honestly, I don't know off the top of my head. It's all muscle memory in my fingers -- I know which buttons to use together to purify, Leech etc, I know which ones are aoe heals, which one is the SS. When I got my new laptop, I had a very hard time assigning my skills, and had to learn from playing the game when my fingers would reach to do something only to learn that I hadn't assigned that keybind yet.
Personally I keep my hotbars the same for Healers regardless of my stance, I do however have all of my Attacks (Stone, Aero, Malefic, Combust) set to Macros using <TT> so that I can always keep my eye on the tank and attack his target.

Just know if you plan to use these macros, anything using 2 words or more in the skills name requires the "" like below.
As for my full keybind, here it is.
Looking at this makes me feel I need to reorganize but at the same time...meh lol. Also when referring to mouse, I use an Azza Alpha with side buttons, so I have one of the side buttons mapped to CTRL.

On PS4 here.
I do this for Scholar, and I love it. Gives me all the buttons I need without any manual set-swapping.
Set 1: Healing
Set 2: Cooldowns, pet commands (Set 2 I pull up with L2 -> R2 and R2 -> L2 using the Extended Crossbar functionality)
Set 3: Attacks
Set 8: General, multi-purpose overworld gaming stuffs (warps, mounts, marking...)
I move from Set 1 to Set 3 using a macro that turns on Cleric Stance and changes the set.
I move from Set 3 to Set 1 using a macro that turns off Cleric Stance and changes the set.
Works perfect for me.
Disclaimer -> I do not main Scholar, although they are my "go to" healer class when I need to heal something.
