Ticket or reference number from your ISP.: None; I called multiple times, and each time, they reassured me that everything on their end and my end were okay.
State/Country: PA/USA

Date/Time (When did the issue start happening, at what specific times does this issue arise, if any.): It started after the initial NA data center problems on December 9th, and never recovered.

Frequency (How often does the issue occur?): This has been happening pretty much constantly, even during non-peak hours. I've gotten disconnected with 90006 a few times, as well.
Content (Which content were you playing in-game when the issue occurred?): Just open-world play, crafting and gathering; I have not been able to do dungeons or anything else due to the lag.
Which in-game World were you playing on when the issue occurred?: Balmung and Sargatanas.
