Ticket or reference number from your ISP: N/A Comcast says no problem, all speed tests show optimal service. No other games experiencing lag. I had lag earlier in the year, it went away completely, then returned since the last maintenance.

Date/Time (When did the issue start happening, at what specific times does this issue arise, if any.): 7-10-11pm Eastern time. (worse around 9pm)

Frequency (How often does the issue occur?): Throughout the evening, most noticeable in instances, but also shows up even when crafting.

Content (Which content were you playing in-game when the issue occurred?): I notice it most in instances, a lot of stop/jerk forward, and LONG stops sometimes. Often happens to several people in the party, but not all. In my own household, it may happen to 2 of 3 of us, or all. (generally all, but not always, on the same modem) Checking other games, no problem.
