None yet. Contacted ISP. Ran some speed tests. Connection is showing fine on tests and working for other games, for streaming vids, etc. A little over 50 Mbps download and over 5 Mbps upload, as per my service plan. When I first contacted my ISP after huge freezes in Snowcloak (see below) the download was around 19 Mbps upload was around 0.24 Mbps. Turns out that even thought I plug an ethernet cable directly into my computer, pathing it through the router first was causing an issue. Directly going from the modem to the computer got speeds around 55.42 Mpbs download and 5.61 upload. So I tested some other things with the new speed and went back into FFXIV for a dungeon roulette that landed me in Dzemael Darkhold, and the huge freezes during boss battles were even worse (again, see below). Another speed test after Dzemael showed the higher speeds were still holding steady.
State/Country
Pennsylvania/USA
Date/Time (When did the issue start happening, at what specific times does this issue arise, if any.)
Had some times over the past month in 4 player dungeons when my healing spells didn't seem to go off (didn't move, target was not out of range/out of line of site), especially on large trash pulls. Usually very infrequent, but a few days ago in my first run of Lost City of Amdapor it was worse. However, earlier today (May 9th), around 5:20-5:35pm EST, I was running Snowcloak for the first time and everything froze (haha) for up to 30 seconds at time during each boss fight.
Frequency (How often does the issue occur?)
Very rarely (see response just above) until now (just finished my first month). I use a direct connection with an ethernet cable rather than wi-fi, set the game to High Priority with Task Manager, have the game cleared with my Anti-Virus software, etc. Today though, it's really bad in group content, some clear cases of minor freezing on trash pulls and huge freezes (from 5 to 30+ seconds) in boss fights, for my Snowcloak clear and my Dzemael Darkhold run for roulette. I basically healed the Batraal fight running in circles and casting Stoneskin and various heals/regens according to what I thought might be happening. Like a double extreme version of the dungeon. Then at 20% left for boss health it was just me running in a frozen game for over a minute. I finally asked in chat if we were still fighting, someone said no and laughed, and then the game sped through the animations from the fight to get back to the present. Can't run any more dungeons tonight, not sure yet if trials would be affected. Someone in the Snowcloak run said they had healed Arum Vale earlier today and the lag made it feel like challenge mode.
Content (Which content were you playing in-game when the issue occurred?)
Tiny blips that delay spells have happened very, very rarely in any level 50 dungeon I've played, but were consistently worse/frequent in Lost City this past weekend. But the blips were small. No game freezes, just a healing spell or two that got lost in the internet somewhere. The big freezes are new. Just ran Snowcloak for the first time today 5:20-5:35pm EST and then a roulette that put me in Dzemael Darkhold between 7:25pm and 7:40pm after checking and testing and contacting my ISP post-Snowcloak. My connection and speed seems consistent and fine and the solo content has no lag at all.

Name of your Internet Service Provider: Comcast

•State/Country: Indiana, USA
•Date/Time: Since the 5/16-5/17 maintenance ended
•Frequency (How often does the issue occur?): every few seconds
•Content (Which content were you playing in-game when the issue occurred?): All content
•Which in-game World were you playing on when the issue occurred? : Zalera
Name of your Internet Service Provider (ISP): Comcast
Ticket or reference number from your ISP: None. Comcast claims no issues exist on their network. Refused to investigate. No other games or sites have this issue and this ONLY STARTED after the NA maintenance.
State/Country: Washington, US
Date/Time (When did the issue start happening, at what specific times does this issue arise, if any.): 5-11pm Pacific time. I entered a previous ticket on this problem back in Jan. It started to recur after this morning's maintenance (5/17/16).
Frequency (How often does the issue occur?): All night.
Content (Which content were you playing in-game when the issue occurred?): Instances as well as open world content though instances were most effected with heavy rubber banding and skill lag delay. I get roughly 11-17% packet loss.
Which in-game World were you playing on when the issue occurred? Diabolos
Traceroute records (optional) (Learn how to generate traceroute record):
Please communicate if there are issues with this maintenance and if your aware that your network had to optimize thus impacting players... if you know Say Something! I transferred from an EU server to get away from the lag and it was great until this latest update on 5/17. Keeping people in the dark about what this maintenance was supposed to do while creating a worse player experience due to it is unfair and undermines the great work many in SE are doing for this game.
Name of your Internet Service Provider (ISP): Comcast
Ticket or reference number from your ISP: Didn't contact.
State/Country: WA/US
Date/Time: 6:30PM PST - ~9:00PM PST
Frequency: The issue occurred randomly, with varying amounts of lag estimated between 1 - 5 seconds.
Content: I was just outside of Moghome and just outside of Falcon's Nest where I experienced it the worst. Long periods where I would be casting spells with no effect or MP usage shown, then all the missed actions would play in "fast forward". Standard lag. It seemed to be less severe when I was at my FC house in The Goblet, but there was still noticeable lag interacting with NPCs and the chocobo stables. The issue tapered off as the night wore on.
Which in-game World were you playing on when the issue occurred? Malboro
Traceroute records (optional): I don't have any, though I did switch to using Mudfish as a test to see if I could route around any trouble spots, but even jumping to various nodes (and with tracert showing that I was getting a completely different route once I left my ISPs network) didn't seem to have much effect. This may not have been the Level3 issue Comcast users have experienced in the past, as I'm fairly certain at least one of the nodes I used wasn't routing through Level3 (unless the datacenter the servers are hosted in peers with Level3 and there's no way around it, of course).
Name of your Internet Service Provider (ISP): Comcast
Ticket or reference number from your ISP: N/A Comcast says no problem, all speed tests show optimal service. No other games experiencing lag. I had lag earlier in the year, it went away completely, then returned since the last maintenance.
State/Country: Maine, US
Date/Time (When did the issue start happening, at what specific times does this issue arise, if any.): 7-10-11pm Eastern time. (worse around 9pm)
Frequency (How often does the issue occur?): Throughout the evening, most noticeable in instances, but also shows up even when crafting.
Content (Which content were you playing in-game when the issue occurred?): I notice it most in instances, a lot of stop/jerk forward, and LONG stops sometimes. Often happens to several people in the party, but not all. In my own household, it may happen to 2 of 3 of us, or all. (generally all, but not always, on the same modem) Checking other games, no problem.
Which in-game World were you playing on when the issue occurred? Ultros.
