Might be hitting a reserved combination. Depending on kbd layout that could possibly also be the taskbar launching shortcut for Windows:
WIN + Shift + number (number represents apps on the taskbar according to order)--launches a new instance of that program. Essentially this could trigger a new instance of ffxiv.exe.
Just want to make sure you are indeed hitting CTRL + Shift + 1 when it happens.
I have actually remapped my win key to be alt because I kept hitting it by mistake.
Might want to double check your key bindings too to make sure it is pointing to a valid macro/action, and that it isn't calling something in Windows or other apps running in the background (like a hotkey for your vidcard or something). Would suck if it was something like a locked system level shortcut to reset GPU...but that would explain it.

I know I'm not hitting ctrl+shift+1, in the game I've tested more than once that and just hitting control plus the tilde key causes the game to crash. Even trying to enter ctrl+~ onto a hotbar keybind in the game causes the game to crash and I can't find any keybinds in windows that has this assignment and it only happens in FFXIV. And I've also found that this only happens in FFXIV after I've hit "Start" on the screen before you load into the character selection screen, so I'm not seeing how it's an issue with anything other than this game.
They may have locked it on purpose...who knows. That key was once the hotkey for opening the console for one of those apps that shall not be named.
Edit:
Forgot about the maintenance...couldn't log in to see what it did on my laptop, so did some googling. Looks like the "`" that is the un-shifted character is sometimes bound as the shortcut to dev console. Just another possible reason for it to behave oddly I guess.

I haven't checked to see if it's still there after the patch, but if it is I'm just gonna bind what is currently ctrl+1 to one of my mouse buttons
Just had a thought...might want to check the hotkeys in CCC. Who knows, could be a funky bind in there if the feature is active. Been a long time since I've tinkered with Catalyst, so drawing a blank on the specific details but I imagine they still have something like the old ATI hotkey poller in there.
Edit:
Just got home and booted the laptop. Can hit CTRL + `, CTRL + Shift + ` (~ is the shift of `). No problems. Nothing loads, no crashes. Game ignores it for me. May be something is up with your specific system configuration?
