Duty Finder Dropping Me

Starting the evening of Nov 10, 2015 I began experiencing an issue with the in game duty finder. Whenever I would queue for a duty and look at the "Duty Status" window I would notice that after 45 seconds or so I would no longer be listed in the party. I am playing as a healer and for healer it says "0/1" and no other player has been listed (tank remains 0/1, DPS remains 0/2) and I have spent 45 minutes in queue with no one listed.

Anyone else experiencing these issues?

Known issues:
An issue wherein the number of matched players for each role may always shows up as a 0 when forming a party in the Duty Finder.
* This is only a display error. The matching process is functioning normally.
