    Do you have Extended Crossbars enabled? If so, you might be jumping to whichever crossbar is extended for a split second as you jump between the left and right triggers - if LB is on that bar, accidental use can happen.

    Either way I'd go through -all- eight of your crossbars and make sure LB isn't on any of them.
