Titan extreme is a good exemple of what should be a difficult instance.
You can pass it the first time you try, no need to embarass your brain with stupid mechanics.

You don't know what titan's leg stomp does the first time you see it (unless you watched the preview video) and the granite gaol tethers are a mystery as well. Tethers have been used in the game to indicate get together (Akh Morn), separate (granite gaol), and intercept (avatar), so it's one of those "you have to test it" things.

Anyway, a lot of those all reflex fights tend to be really easy. That's not necessarily a bad thing, just something to watch out for. KOTR extreme will probably be pretty easy, but most extremes aren't that tough anyway so whatever.
