Ah, so the vulnerability comes from the adds, which absolutely have to be killed under the nukes, so any DPS positioning the vital adds will pretty much get unavoidable stacks. That makes a lot more sense. That would also explain why I didn't get any stacks. I contributed DPS to the adds, but I never had aggro on them. Thanks for this!

I'll do some mitigation before the add phase and I'll save some AF for indo after the nukes and that should fix it.
