I experienced a similar issue during the Foundation Day event. Although I received the tri-color flag avatar on the Lodestone, I did not receive the Grand Company salute avatar. I completed all of the requirements and received all of the achievements available but the avatar is still missing from my profile.

My guess: The avatar probably didn't appear on my Lodestone page because I joined a Grand Company just six hours before the conclusion of the event (and, of course, the Lodestone takes forever to update character history). By the time my achievements updated, the event had already ended.

It's disappointing to see that SE only offered the Foundation Day avatar for the first few days of the event when Foundation Day ran nearly two weeks after patch 1.19 was implemented. This is something that should have been awarded to every character that participated in the event. It's also disappointing to see that the salute avatars are only available to those characters who joined a Grand Company, and received the achievement, during the Foundation Day event.

I guess I'll just have to rush through the upcoming All Saints' Wake so I know my achievements move to the Lodestone correctly and hope that I can complete everything available before SE throws a patch into the middle of another event.

Update: When I logged onto the Lodestone this morning, after collecting the first three pumpkin heads from All Saints' Wake, I took a moment to look over my achievements; just to make sure they were updating correctly.

At that time, my two most recent Lodestone avatars were the tri-color flag followed by the first pumpkin-head avatar. When I logged back on about four hours later the Grand Company salute avatar had appeared between these two.

Obviously, I'm confused.

Either it takes nearly two days for the salute avatar to appear on a character's Loadstone page or someone fixed whatever issue I was having. I'm hoping it was the latter and that those players who missed out of the tri-flag avatars, due to patch 1.19 or Loadstone issues, will see their issue resolved too!
