I took a look at FFXI's Nyzul Isle Investigations, its a random dungeon area with tough enemies that actually drop loot that's equal to that difficulty.
Well, if you don't heal they are a threat, if you don't stun they are a threat, if you don't kill the right one first they are a threat, so you can't say they aren't a threat, it's just the content has been out long enough to know what to do and not to do. You can't really spice that up because once you get to a certain IL and understanding of a dungeon you can't really make it difficult unless you lower your own IL and just make AOE spam impossible to pull off.
