Make the size of an enemies damage circle, square,triangle, whatever shape that actual size. It is bullcrap when we are clearly out of the range and we get hit. I have heard a lot of people complain about it.

I may be wrong but the way I have seen things is it registers if you are safe or not when you began to move out of the aoe..NOT the fact that you see yourself out of the aoe.
many times i too have clearly moved out of an aoe..well away and the graphic is still on the ground but I got hit simply because i BEGAN to move too slowly. You can literally be a mile away from the graphic and still get hit if you paused for that millisecond (<--literally) when it initially began, which is why it seems deceiving
(that may not be your issue..but thats exactly how it works for me)

I don't think I've ever run into this. I'm pretty sure that this is a ping issue more than anything else.
It's server side lag due to the game prioritizing what the server says over what your client says. i.e. The server says you got hit by the aoe, your client says you were waaaay out of it, but the server wins out always. Same reason why you can see Hollowed Ground or Benediction go off, see it in your chat log, see that the ability is on cool down, but still die.
It won't change until SE realizes server side structuring is bad for a game.
