I use an Arachnid Sicarius (link to picture - people with spider phobias should be aware that Arachnid's logo is a spider and is displayed on the mouse).
It has enough buttons for my taste, all of them programmable. Since I never really change DPI settings and just find it annoying when I accidentally click that button and end up with a slogging-through-molasses mouse speed, I don't have a button with that function anymore xD
For hotbars, I mainly use slot 1-6 (well, my hotbars start on the key to the left of 1, which on my keyboard is §, and end on 5, but it's still 6 slots) and use one mouse button for ctrl and one for alt, to give me a potential of four hotbars. Less used skills are non-keybound and I just click on them to use them (summons, limit break, raise, etc.).
I also have one button set as F2, to give me a convenient shortcut to the tank for when I heal, and one key to select my focused target.
As for keyboard, I use the A4 Tech X7 G800V (picture). It's a cheap one, so the keys are a bit... rattly if I shake the keyboard, but it's functionally sound, and the best of all is that it has the thumb macro keys, which I use for an additional hotbar (macroed to ctrl+shift+#, iirc) with more important less-often-used skills, such as protect, stoneskin, stances (cleric, tank/offtank), mudras and such things. Still haven't seen another keyboard with more than three thumb keys. (I've also moved one of the rubber keys from the arrow pad to my § key and one to my 5 key, to give me a tactile feedback of where I am which means I have to look down at the keyboard less often to avoid hitting the wrong skill/spell.)

Currently using a Razer Naga Molten, though during a couple of moves some months back, whoever moved my computer must have been really rough on the mouse, because something inside is broken and sliding around, causing the mouse to malfunction every now and then. But before that, the scroll button on the mouse will periodically stop working. I'm definitely going to replace it when I have the cash to, possibly with a Logitech G600.

I love things where the LEDs can have their color changed so much.

Using a Logitech G502 Proteus Core. After 3 Naga MMO mice that broke after 3 months of use I will never ever go again to Razer low quality products.
Mouse: Logitech G303 Daedalus Apex. Small, light-weight and extremely accurate sensor.
Keyboard: Ducky Mini. Small and mechanical. No nonsense performer.
Controller: Logitech F710. Wireless with long battery life. My preferred layout (Dual Shock) but with the more common Xbox button icons.
I mostly use mouse+keyboard but for some tasks such as mining I usually switch to the controller.

Does anyone have experience with these mice and can share their opinions:
Mad Catz MMO 7
Gamdias Zeus
Roccat Tyon (cat on the palm is off putting)..
Utechsmart Uranus
I keybound all XIV commands to mimic the same keyboard-only controls I have on FFXI. Mouse is overrated.

I ended up getting a CM Storm Inferno, works great and no hand pains
