I'd like to commend people after they have left a duty. PVP is the biggest example because everyone is in a rush to get out to re-join again. I don't blame them for doing this because the queues are always long but I'd still like to give them the commendation all the same.

The unspoken trend is that healers and tanks will get the lion's share of the commendations by default. As these roles are seen far more vital than a role whose focus is damage (and a bit of support in the case of Bards and Machnists). I primarily play DPS, yes, I've dabbled in healing and tanking, but I prefer the role of the damage-dealer to keep stress levels low. Because of my preference, and knowing that tanks and healers are going to receive most of the commendations, I look at my fellow damage-dealers. Unless they perform so horribly, or have a terrible attitude, they will get my vote.
Now there have been times where I wanted to give everyone in my party a commendation, but the best I can do is thank everyone for the group, give out my commendation and go about my merry day.
I LIKE the fence. I get 2 groups to laugh at then.

usually if i can't decide on who should get the comm, then i won't give one out at all... to be fair of course

Something I've noticed is that during 4man dungeons the tank is usually more likely to get more commendations, and during 8man ones the healers are more likely to get more. lol just from my experience from being a tank and my healer friends. Though overall the one that best communicates/most positive (or is most helpful especially to first timers) is also one of the biggest factors on who gets the most commendations over everything given they don't play terribly to offset that haha.

I will ALWAYS commend my fellow healer on an 8-man party. It's Healer Etiquette for me.Something I've noticed is that during 4man dungeons the tank is usually more likely to get more commendations, and during 8man ones the healers are more likely to get more. lol just from my experience from being a tank and my healer friends. If everyone parsed you'd get more comms as dps assuming you werent shit.
Life's a tease.

I think the commendation system is fine as it is, if you could give everyone a comm, it wouldn't a nice surprise seeing a comm after a duty, but be expected. You give it to the player which left the most positive impact on your run, whether that is busting out high DPS, tanking lots of mobs, healing through a dicey situation, making them laugh, being helpful, giving them buffs etc.
Yeah, sometimes you wish you could give it to a whole party, but ultimately you have to choose which ONE left the biggest impact. Me personally, explaining fights and being helpful to new players will earn you a comm from me, otherwise I will be keeping an eye on the other players and see which one I think should have one (if anyone at all)
Never expect comms, they are a nice surprise for being a good sport, just play well and have fun.

I think I have gotten more commendations as a DPS from entire parties than I have healing. ^ ^;; I was getting a lot as a BLM. But yeah, tank and healer should have each others backs, as should the DPS. Unless, like someone mentioned, a killer stunt was pulled. (Or if the entire party is quiet and moody. >.<; I don't usually give comms then.)
As a DPS I find myself rarely ever getting commendations, even if I try my best. Although with the recent addition of the TP bar, I can goad far better and that's netted me a few more commendations than usual. I got more commendations leveling my Conjurer to 20 something than I did in 3 months before this TP bar was introduced, even though I was doing barely anything but clicking a few buttons (but I guess that can be said about every class during low level play).
I just see it as an incentive for people to play the more needed classes, although I've started always giving the commendation to the other DPS if they manage to do good DPS. Maybe I'll stop being lazy and finally level a WAR or DRK for those rewards.
