    It evens out a little bit when melee are required to move out of range. Proper use of instant procs can help alleviate movement as well as knowing the mechanics for when you can go optimal and when you dial it back so you -can- go optimal when your windows open.

    No different than any other class.

    If you include the movement in any fight, assuming equal gear and skill, a Melee will always outdps a caster (outside of any buffs from other classes).
