Materia Conversion Popup Widget Bug

So far, each time I've created a materia from a piece of equipment, the widget that informs me what type was made does not disappear from the screen, any key presses do not remove it and it also disables all ctrl and alt hotkeys and disables menu access either from the keyboard or the micromenu (I have to directX error myself out of the game and log back in to fix it)

In order to make sure that we have sufficient information for a thorough investigation, it would be a great help to us if you could list the equipment that you used when you experienced this, as well as the type of materia that you were trying to create. In addition, the following details would improve our ability to try to recreate what you have been experiencing:

Platform:
Specify which veersion of Windows was in use to play the game.

ISP:
Name of ISP you use to connect to the Internet.

Type of Internet Connection:
Specify the type of connection. (dial-up, DSL, CATV, Optic Fiber, Other)

Internet Connection Speed:
Specify the speed of internet connection. (Example: 768Kbit/s, 5Mbit/s)

Date & Time:
Date and time of when the issue occurred. Please be sure to state the time zone of your region. (Example: Pacific Time = PST/PDT)

Frequency:
State how frequently the issue occurs. Please select from the following.
1.Always
2.Sometimes
3.Once (Did not reproduce)
4.Unknown

Character Name:
Which character did the issue occur with?

Race:
Race of your character when the issue occurred.
(Elezen M, Elezen F, Hyur M, Hyur F, Lalafell M, Lalafell F, Miqo'te, Roegadyn)

World:
World where the issue occurred.

Class/Skill Rank:
Class and skill rank of your character when the issue occurred.

Physical Level:
Physical level of your character when the issue occurred.

Area and Coordinates:
Area and map coordinates where the issue occurred.

Party or Solo:
If you were solo when the issue occurred, please write "Solo". If you were in a party, please write "Party". With alliance, please write "Alliance."
*If you wrote "Party" or "Alliance", please also include the number of members.

In-Game Time:
Please list the in-game time that the issue occurred.

Steps:
Detailed steps on how the issue occurred.
*Please try to include the following details.
•What you were doing when the issue occurred. (Example: Battle, mission, quest, crafting, synthesis, etc.)

So far this has occured each time I have created materia. The first time was during the forging the spirit quest and used a Dated Dodoskin Cesti which created a Touch of Serenity Tier 1, the second time was during no quests and used a Cotton Tunic (Blue) which created a Bloodwall Tier 1, the third time was a few minutes ago, during no quests with a Brass Choker and created a Vitality Tier 1, I was not attempting anything specific.
Platform: Windows 7
ISP: TWC Roadrunner
Type of Internet: Cable
Speed: 1.4 Mbits/s
Time Occured: Approx. 12AM EDT and 12:50PM EDT of the date of this post
Frequency: As of this writing I have converted three pieces of materia, it has happened every time
Character Name: Jho Stumps
Race: Miqo'te F
Level: 24 the first two times, 25 the third, PGL all times
Solo play
Unable to remember the exact time for the first two times, the latest conversion occurred around 13:00
Every time this happened I was merely standing around in field (Western Thanlan the first two times, Gridania the third), no macros were used and no error messages appeared, but the error prevented me from accessing menus and many hotkeys
Addendum: I am able to use the /logout command to log off and back in to the game and that removed the widget

I just tried again at about the time of this posting, first with a pair of Dated Cotton Kecks which turned into a Loresman's Wit Tier 1 and then a Dated Brass Earring which turned into a Piety Tier 1, I did them back to back and the Loresman's Wit widget remained on screen but the Piety widget appeared over top of the Loresman's Wit widget and then disappeared, leaving only the Loresman's widget on screen
Edit: Kept trying with four Dated Brass Rings, each produced a materia and popped a widget up on the screen over the top of the Loresman's Wit, and then promptly were removed from the screen after a couple seconds so the problem seems to stem from the first widget but none of the successive ones

It happens everywhere anytime you create a materia. Happened to me 4 times in a row. You become unable to open the menu or any other windows. can only /logout to fix.

I'm really pleased that I'm not the only one this is happening to, I thought it might have just been me lol. The good news is that you can convert multiple pieces at once and only have to do the logout fix after you are finished, looks like the stop-cap solution until the bug is remedied

I have made sure to submit all of your information to the development team, and they will make sure to thoroughly investigate this matter, based on your report. Unfortunately, I cannot guarantee that a direct response will be posted on your thread, but I do recommend checking here or here for any details regarding the development team's investigation. I also suggest contacting the SQUARE ENIX Support Center, just in case they may be able to find the source of this situation. Thanks again for your report!
