Go to North Shroud talk to the weapons smith guy in the village above Gradania and it pretty much explains from there
Also google some guides but basically its some running around, run some dungeons and kill Hard Mode Ifrit, Titan, Garuada etc....
Be sure to have completed the story to the proper point, and make sure you've completed all of your BLM job quests up to 50, then talk to Nedrick Ironheart in Vesper Bay 12,14 Western Thanalan.

It actually starts at Nedrick Ironheart in Vesper Bay if you've never obtained a relic weapon before. 12,14 in Western Thanalan.
It's been a while since I did my Relic, only bothered to do it on Warrior lol, since 3.0 my Paladin and Dark Knight are using weapons from Ishgard and markets above i100.
Do be warned that the queues for the old primals and a lot of the dungeons are extremely long nowadays, for DPS especially. Doing the book stuff I've frequently been in queue for 45 minutes to an hour.
I've done the relic quest for a couple of my DPS jobs in the last 2 or 3 weeks, and most of the queues were under 5 minutes. I did once (only once, out of 10 trials and 2 dungeons) have a long queue, which I dropped out of after about 40 minutes, but then got in almost immediately when I re-queued for it a few minutes later.
Anyway, the quest involves 5 trials (Chimera, Hydra, Ifrit (Hard), Garuda (Hard), and Titan (Hard)), a dungeon (Amdapoor Keep), and a few open-world mobs. You'll also need to make or buy a crafted weapon with two materia. In the case of Black Mage, it's a Sanguine Scepter with two Savage Might III materia.

It's a little hard to just answer what you're supposed to do, though, unless we know which step of the process you were on when you got lost, since this quest has several steps involved.

I have the quest already. And I did a bunch of hard mode primals. The point I'm at now in the quest is what Niwashi described. It tells me I have to get a blm weapon and meld it. I don't know if it can be any weapon? Are how to meld means? Niwashi said I need to get the scepter weapon so I guess I'll just buy it off market board then meld the materia. And then we'll see what happens. I'll keep you guys updates thank you
The weapon for Black Mage needs to be a Sanguine Scepter and the materia needs to be Savage Might III (you'll need two of that materia). And you'll need to find a high-level Goldsmith to meld them together for you (unless you happen to get lucky and find the scepter listed on the Market Board with the right materia already on it). The main market area of any of the three main city/states is usually a good place to find crafters willing to help you with the melding part. Just head there and ask in the /shout or /yell channel if there are any goldsmiths around willing to help out who can meld level 50 items.It tells me I have to get a blm weapon and meld it. Once you find someone willing to help, to have them meld the materia onto the scepter, right-click on them (or select them and use the square button on PS3/PS4 controller) and choose "Request Meld" from the list of actions that comes up. You'll get a window where you'll need to choose the equipment (Sanguine Scepter) and materia (Savage Might III) that you want melded.
Edit:
Almost forgot: You'll also need to have a piece of Grade 3 (or higher) Carbonized Matter for each of the two melds.
