According to the skill description, it makes one of the next three appraisals not increase wear. I haven't tested this extensively, but I assume it chooses one of those steps randomly and you won't know which it is until you get the hit that doesn't increase wear.
That said, the best you can do with it is to know you're getting it on the third hit and use Instinctual Appraisal for free, but that saves you 15 wear and for the same GP cost you can just use Utmost Caution three times. So the short answer to how do I use Deep Breath is, I don't.
